This is Scott Adams idea of what happened with the Lucent Logo (and yes it is called the "Circle of Innovation").
However my theory needs a little insider knowledge of the Lucent buildings. The stalls in the toilets on the ground floor of the Holmdell building in New Jersey have circular chrome door handles. On the inside of the door in the first stall of most WCs the paint has been worn off around the handle in exactly the same circular way as the Lucent logo. I can only guess that after 3 days of trying to come up with a design a tired executive nipped in there for a number 2 and hey presto! "guys we can go home I've found the log(o)"
